Reform of the Medicare Physician Fee Schedule is Needed Now

ADVION supports the adoption of comprehensive reforms to the Medicare Physician Fee Schedule payment system and urges Congress to pass the Medicare Patient Access and Practice Stabilization Act of 2025 (H.R. 879). Over the past 20 years, provider reimbursement has declined by 30% when adjusted for inflation, placing unsustainable pressure on clinicians and threatening patients’ access to timely, high-quality care.

The PURE Act (H.R. 2477/S. 1168)

ADVION urges Congress to pass the Portable Ultrasound Reimbursement Equity (PURE) Act (H.R. 2477/S. 1168), which would recognize portable ultrasound procedures and provide the same transportation reimbursement that Medicare currently allows for portable x-ray services. This bill removes barriers to care, providing physicians with timely and critical diagnostic care without unnecessary and costly hospital visits.

Support Digital Telehealth and Permanent Medicare Telehealth Extension

ADVION urges Congress to pass H.R. 1614 to permanently add OTs, PTs, SLPs, rehabilitation therapy assistants, and audiologists as Medicare telehealth providers in the U.S. Code,  which will allow them to continue providing rehabilitation therapy services to Medicare patients via telehealth.

Physician Fee Schedule Reform at Risk - Urge the Senate to Act 

Congress is continuing its work on reconciliation legislation and a crucial provision was dropped from the Senate version of the bill.  The Senate Finance Committee dropped the Physician Fee Schedule inflationary update for 2026 and beyond.  This is a key provision for clinicians in that it would provide a positive payment update.  While we expect the Senate's legislation to continue to evolve, it is imperative that Senators hear from their constituents to advance meaningful reform of the Physician Fee Schedule.

Contact your Senators today to urge them to include reforms to the Medicare Physician Fee Schedule.

Include LTPAC in Health IT Incentives

ADVION joins the authors of the National Academies of Science, Engineering and Medicine (NASEM) 2022 report in urging the federal government to provide financial incentives for health IT adoption that supports health information exchange to enhance person-centered longitudinal care in the long-term and post-acute care (LTPAC) sector.

The Disadvantages of Medicare Advantage

ADVION supports the Improving Seniors’ Timely Access to Care Act (S. 1816/H.R. 3514), which would reform the use of prior authorization in the Medicare Advantage (“MA”) program. This legislation would help protect patients from unnecessary delays in care due to overuse and misuse of prior authorization in MA. It would streamline and standardize the use of prior authorization in many situations and provide much-needed transparency for rehabilitation patients.

Protect Medicaid

ADVION urges Congress to oppose any Medicaid cuts that would jeopardize access to nursing home care and critical services for seniors and individuals with disabilities. Medicaid is the primary and largest single payer for care provided in nursing homes, covering the cost of care for approximately 2/3’s of nursing home residents. Cuts to Medicaid will lead to increased state financial burdens, reduced provider reimbursement, and a loss of essential services for our nation’s most vulnerable populations.
